Rules for Participation and Use of Aggregate Data

We would like to receive just one set of responses per participating organization.

Only those organizations which participate in the survey will receive a copy of the complete results.  For this reason, you'll be required to complete a brief demographic section identifying your name, organization name, e-mail address, city and state.

All demographic and response information will be kept anonymous. No individual data will be shared.

The results you receive as a participant will contain the aggregate data, along with a narrative interpretation of the findings.  We hope to have the results back to you by April 15, 2010.  Lynn Smith-Cronin and myself (Sue Altman) may prepare additional analyses and submit articles for publication regarding the survey findings - but again, no individual data will be mentioned or shared.
